Career Exploration
Is this career for me? One way to determine if the field you are considering is a good fit is to job shadow or observe. Below is the information you need to know and the application to do so. Please note that application form processing may take up to two weeks during high-volume seasons, such as spring break, summer break, and winter break. Application approvals are based on the number of requests received, census, staffing and space restrictions.
NOTICE: We are not able to take observation applications after December 2, 2024 as our observation opportunities have filled up from December 2, 2024, to January 5, 2024. If you would like to be put on a waiting list, please complete the application to be considered a candidate for our waiting list for holiday break opportunities. Please complete the application if you would like an opportunity for any dates after January 5, 2025.
Observer Information
An observer is a person who is in the facility to shadow a specific person(s). The observer will have no direct physical contact with the patient, nor would they, in any way, direct or participate in the treatment of the patient. This person is in the organization to observe only. You must be at least 16 years of age to observe in our facilities. A typical observation experience in any Logan Health department is 4-8 hours. Communications from the Student Department are done via email so please check the email you provide in your application for any updates, approvals or any documentation needed to complete your request.
*This program is for career exploration, not obtaining clinical experience.
**Observers may be scheduled at any Logan Health facility for no more than 20 hours in a rolling 12 month period.

Minor Observer Agreement
Minor Observer Agreement: If you are under the age of 18 you will need to have your legal parent or guardian complete the Minor Observer Agreement and upload it to your application.

- Take a headshot where your head takes up 60% of the frame.
- Avoid selfies.
- Avoid extreme angles and use of filters.
- Select a simple background.
Minor Observer Agreement (If you are under the age of 18)

- You must be at least 16 years of age to observe in our facilities.
- If you are under the age of 18 you will need to have your legal parent or guardian complete the Minor Observer Agreement and upload it to your application.
Providing your vaccinations is voluntary. If you are willing to volunteer your vaccinations, please provide and upload the following.
MMR (measles, mumps, rubella):
*You are not required to provide proof of vaccination but if you do not do so, we will consider you to be unvaccinated.
- *Provide one of the following:
- 2 official records receiving vaccinations
- Positive titer results for each measles, mumps and rubella proving immunity
Varicella (chickenpox):
*You are not required to provide proof of vaccination but if you do not do so, we will consider you to be unvaccinated.
- *Provide one of the following:
- Record of 2 vaccinations
- Positive titer results proving immunity
Diphtheria, Tetanus, and Pertussis (DTaP, Tdap):
*You are not required to provide proof of vaccination but if you do not do so, we will consider you to be unvaccinated.
Provide one of the following:
- Under 18 years of age:
- *Provide one of the following:
- 4 doses of DTap(one dose must be given on or after 4th birthday)
- 1 dose of Tdap
- 18 years of age or older:
- One dose of Tdap (Tdap only – does not include DTaP or Td or tetanus)
***Injunction was lifted on Montana House Bill 702. In response to this change, the following changes are occurring at Logan Health.
Do I need to be vaccinated to be a student or instructor at Logan Health?
Vaccines are not required.
What happens if I am not vaccinated or decline to provide proof of vaccination?
A representative from Logan Health will contact you via the phone number you provide to have an individualized conversation.
